We are located in Manchester, NH and provide in-home and in-community services in:
southern Merrimack County
eastern Hillsborough County
western Rockingham County
If you would prefer to submit your information on paper or via email, please email admin@ababc.org for a referral form.

Copy of insurance cards, front and back
Treatment consent (will be emailed to you)
Required documents:
Incident reports from past year
Any data that has been collected
Past diagnostic assessments/evaluations
Current service agreement
Guardianship letters and orders, if applicable
